CSDA Member Dentists: If you took courses for continuing education and had your ADA card swiped, the credits will be added to your online transcript by June 15th. If you did not have your card swiped, you should have picked up a CEU Verification Form at the Information Kiosk so you could note the codes given at the end of the classes. This form does not get submitted back to the CSDA – you need to keep it on file for your records. A carbon copy was provided if you need to submit your credits to the AGD.

Auxiliary Staff: If you took courses for continuing education, you should have picked up a CEU Form at the Information Kiosk and noted the codes given during the classes you took. You must retain this form for your records.
